Page 1, client and pet: contact details, where to park, an emergency contact, the pet’s breed, weight, and vet, rabies date, health problems, behavior, and the groom they want.
Page 2, grooming agreement: the owner initials each part. It covers mats and shave-downs, senior or nervous pets, bites, emergency vet care, fleas, and photo permission. There’s space for your own cancel, late, and payment rules, and lines to sign.
